This program delivers sensitive, timely and portable services to individuals ages 16 and older who are struggling with a mental illness and/or concurrent disorder. Services are based on a psychosocial model of care which facilitates consumer’s full and active participation in the recovery process and promotes independence.
How to be referred
Please click the following link www.accesspointnorthwest.ca in order to complete an on-line referral form for case management services. Clients may self-refer or may be referred by another person/agency (ie. health care provider, family members etc.)
